Each year all faculty members must submit a Faculty Activity Report (FAR) to the department chair/head. Due dates are set by individual departments. A full updated CV must be submitted with the FAR. These documents should be submitted electronically.

The reporting periods to be included in the FAR are by calendar year. Include activities and accomplishments across the spring, summer and fall semesters of the previous calendar year.

Annually, COS departments shall electronically submit the individual FAR from each faculty member to the Dean’s Office by the first Friday of June. Department administration should place the FAR and CV of each faculty member into individual folders in the departmental FAR folder on the NAS. Department head comments and the performance evaluation summary should be placed in the DH_COMMENT folder in the FAR folder.
